Required Documents

  • Application through the American Medical College Application Service (AMCAS) a centralized application service for applicants to first year classes at participating U.S. medical schools. The complete application must be processed by AMCAS between June 1st and before December 15th of the year prior to admission. We urge candidates to submit the AMCAS Web application with enough time in order to be considered before December 15th. Our institution does not require a secondary application.
  • Qualified applicants will receive an email from WebAdmit for AMCAS with a link to the “SJBSM Activities Gateway” to begin the supplemental application. Please be sure your email account is set up to accept emails from donotreply@webadmit.org.
  • Non-refundable certified check or money order for the amount of $100.00 payable to San Juan Bautista School of Medicine or you may use a credit card and pay electronically. To do so use this form and send it to bryan.mills@sanjuanbautista.edu
  • Prerequisites courses listed as “in progress” on the AMCAS application must be completed by matriculation. Official transcripts with conferred grade will be requested upon admissions.
  • Official results of the Medical College Admission Test (MCAT) with a minimum score of 495 sent by AMCAS. The test must not be older than three years.
  • Two (2) letters of recommendation* from science professors, sent via AMCAS Letter Service.
  • One (1) letter of recommendation* from a community leader or volunteer work specifying any community and/or volunteer work sent via AMCAS Letter Service.
  • Altus Suites - three-part online assessments of non-cognitive skills (CASPer, Snapshot, and Duet). For more information, please access https://acuityinsights.app/

Any one of the following will satisfy the academic letters of recommendation requirement:

  • Committee letter (required if your college/university has an officially designated committee or advisor). Authored by a pre-health committee or advisor and intended to represent your institution's evaluation of you. A committee letter may or may not include additional letters written in support of your application.
  • Letter packet: A packet or set of letters assembled and distributed by your institution, often by the institution's career center.
  • Faculty letters (required if the college/university you attended does not have a committee or advisor).
